ImageWe’re asking cottagers to fill out printable downloadable information form PDF forms. Online information forms will be available soon.

The first form, “General Comments,” should be filled out and asks for overall observations, comments and evaluation of the waters in your cottage community. Information will be collected on an ongoing basis and summaries published on regularly.

The second form, “Stop Log,”  is optional and is intended for those cottagers with boats. It has spaces for comments on water quality, debris, industrial pollution, wildlife and more.

Participants’ names and contact information are needed for verification purposes but will remain confidential and will not be made available to any third parties.
